AI in Pharma: Autopilot Is Not the Same as Removing the Pilot

While much of the current mainstream AI conversation focuses on the pace of adoption and scale of productivity gains, in regulated industries like pharma, the more consequential issue is control. Across the clinical development lifecycle, AI applications are being proposed and introduced at a rapid pace, with regulatory agencies actively exploring how AI can support elements of the regulatory process. The momentum and potential are real, but a key challenge remains in ensuring that automation enhances decision‑making without eroding governance, compliance, and human accountability.
This is not unique to pharma. For example, debate recently emerged in the defense sector, where discussions between the U.S. Department of Defense and leading AI developers have centered on the safeguards needed for responsible AI use. In pharma, these principles play out in how AI systems are being designed, used, and governed. Here, one of the most important, and often overlooked, distinctions is between consumer‑grade and enterprise‑grade AI models.
Consumer-grade and Enterprise-grade AI models
Many widely advertised efficiency-focused AI consumer models rely on large-scale data ingestion and continuous model improvement across users. In contrast, enterprise models, particularly those designed for regulated industries, are often architected differently. Ideally, they should operate in isolated environments, avoid cross-client learning, and apply more restrictive data retention and retraining policies.
Sponsors should be deliberate about partnering with service providers who understand these differences. It is not enough to adopt AI. It is critical to understand how data is retained, processed, and potentially used for model improvement. Providers need to demonstrate depth in model architecture, mature AI adoption frameworks, robust data handling policies, and governance controls that extend across the full AI-enabled service delivery process.
That raises practical questions. When sponsors use AI tools, is their data used to improve shared models? How is cross-client learning prevented? Who owns derivative outputs? How clearly are data use boundaries defined?
In regulated environments, data is not just an operational input. It is intellectual property, competitive strategy, and often patient-sensitive information. The distinction between using AI and training AI is material.
Leveraging a tool to support a defined task is fundamentally different from contributing proprietary assets that strengthen a vendor’s broader commercial model.
Domain Knowledge is Paramount
There is also a capability dimension that deserves thoughtful scrutiny when considering how AI is introduced in delivering critical services such as regulatory strategy or regulatory submissions. Many AI entrants are technology-first, technologist-led organizations. Their advancements are meaningful and often impressive. However, they may not have deep regulatory domain expertise or firsthand experience navigating agency meetings, defending endpoints, or managing global submission strategies. An AI model can generate highly polished outputs. Regulators, however, expect contextual judgment, precedent awareness, and risk mitigation that extend beyond technical proficiency.
It is like having a world class design firm prepare a complex clinical protocol. The layout will be sharp, and the language polished, but without hands on regulatory and therapeutic expertise, the core arguments may not withstand agency scrutiny.
“Speed is a Metric, Not a Strategy”
There is another critical dimension to this topic, which seems to be largely sidelined in the most prominent industry dialogue. At this time, much of the industry narrative around AI is framed around percentage time saved. Faster drafting. Faster analysis. Shorter cycle times. Efficiency matters, but speed is a metric, not a strategy. The real question is where AI meaningfully enhances decision quality across the development lifecycle.
Used appropriately, AI has clear value. In early discovery, it can accelerate target identification and compound screening. In trial design, it can optimize inclusion criteria and support site selection. In clinical operations, it can forecast enrollment and flag anomalies. In safety and pharmacovigilance, it can strengthen signal detection. For document generation, it can improve consistency and reduce avoidable variability. All of these are valuable. Yet the value assigned to AI in a regulated environment depends on more than velocity. It depends on governance, domain expertise, clear data boundaries, and thoughtful integration into scientific and regulatory workflows.
I think about it like autopilot in aviation. The autopilot reduces workload and improves precision; however, no airline removes the pilot from the cockpit. When turbulence hits or conditions change, judgment and context matter. Accountability controls the cockpit.
In pharma, the stakes are just as real. Decisions around AI adoption and partners should not be based solely on claims of acceleration. The more important questions center on model versioning and control. Who governs updates? What guardrails are in place and how are they protected? What happens when those guardrails are tested, overridden, or modified?
Equally important is how multi-client data is handled, where the vendor operates from a jurisdictional standpoint, and what regulatory and therapeutic subject matter expertise is embedded both in the model and in the team designing and maintaining it.
Human-in-the-Loop at the Center
In our industry, raw velocity without governance is a recipe for a nose-dive. Stability and accountability are what ensure a safe landing time and again with every program.

AI will automate elements of process. But it will not automate accountability or regulatory and scientific judgment.
